Effective communication with the family and clinical coordination **Job Description** Provide home-based nursing care with a clinical and human-centered approach, ensuring patient safety, continuity of care, and effective communication with the family and clinical coordination. Perform basic assessments, follow medical instructions, administer medications safely according to the patient’s profile and applicable regulations, carry out nursing procedures within scope of competence, maintain clinical records, and promptly report incidents, while complying with biosafety and confidentiality requirements. **Main Responsibilities** * Assessment and documentation: vital signs, pulse oximetry, capillary glucose (as indicated). * Safe administration of medications (via prescribed route) according to protocols. * Basic/intermediate wound care and skin care (as applicable). * Basic management of devices (e.g., urinary catheter, oxygen therapy, ostomy) when required by the service. * Patient/family education on prescribed care. * Clinical log/documentation and communication of clinical alerts. **Requirements** * Bachelor’s degree or technical diploma in nursing (as specified for the position), with valid documentation. * Clinical and/or home-based experience (preferred). * Availability for travel and assigned schedule. * Commitment to quality, respectful treatment, orderliness, and communication. **Inclusion Commitment** This vacancy complies with the Federal Law to Prevent and Eliminate Discrimination.
