We combine technology, continuous support, and preventive health plans to ensure our community lives with autonomy, energy, and well-being We seek an Internal Medicine Physician with specialization or subspecialization in Endocrinology, Metabolism, or Diabetes who wishes to take ownership of our Type 2 Diabetes Mellitus (T2DM) program. Requirements Your Daily Missions 1\. Program Leadership and Evolution Design, update, and monitor adoption of T2DM clinical protocols (risk stratification, control alerts, and case escalation). Serve as mentor to the follow-up team (general physicians, nutritionists, educators), resolving their questions and auditing clinical quality. **Analyze program data:** measure population-level glycemic control, prevent complications, and propose improvements based on real-world outcomes. 2\. High-Impact Clinical Practice Manage (in-person and virtually) the most complex cases within the program. Interpret and validate Longevity Studies (LS) and metabolic health reports for our patients. Coordinate patient transitions and home returns following hospitalization or acute decompensation events. 3\. Community and Outreach Continuously train the healthcare team on the latest trends in T2DM management for older adults. Co\-create digestible and authentic medical content (written or audiovisual) to educate our patients and their families. How Will We Measure Success in This Role? Keeping program protocols updated, agile, and flawlessly executed by the entire team. Ensuring our T2DM patient community achieves optimal metabolic control, reducing hospital admissions and severe complications. Being the present and accessible clinical reference that provides cohesion and direction to the program. What We Offer **Flexible Hybrid Model:** In-person consultations for complex cases and delivery of Longevity Studies; remote video consultations and schedule management. **Contracting Scheme:** Medical honorariums (full-time equivalent: 8 hours per day). **Working Hours:** Monday to Friday, 08:45 AM to 05:45 PM (with a 1-hour lunch break). **In-Person Location:** (Santa Fe / La Ponderosa area). 2 years
