SIMPLE COPY OF THE COVER PAGES OF EXECUTED CONTRACTS, PURCHASE ORDERS OR WORK ORDERS, SUBCONTRACTS, AND DELIVERY CERTIFICATES OR SETTLEMENT DOCUMENTS IN WHICH THE CANDIDATE HAS PARTICIPATED AS A PMP. CERTIFICATE ISSUED BY THE PROJECT MANAGEMENT INSTITUTE (PMI) CERTIFYING THE CANDIDATE AS A PROJECT MANAGEMENT PROFESSIONAL, VALID DURING THE EXECUTION OF THE WORKS ACCORDING TO THEIR START AND END DATES. (LETTERS OF INTENT STATING THAT THE CERTIFICATION WILL BE OBTAINED DURING PROJECT EXECUTION WILL NOT BE ACCEPTED.)
